Roles & Responsibilities
- Monitor telemetry continuously and respond promptly to rhythm changes and alarms
- Administer medications and IV therapies safely, including heparin drips as ordered
- Provide compassionate direct patient care for adult and older adult populations
- Document all care and assessments accurately in EPIC EMR and complete required checklists
- Use specialty equipment such as gait belts, geriatric chairs and PT equipment to support mobility and safety
- Collaborate with interdisciplinary team members to develop and implement patient-centered plans of care
- Support skilled nursing and rehabilitation goals to help patients progress toward discharge
- Maintain BLS and ACLS competencies and participate in unit-level quality and safety initiatives
Qualifications
- Current BLS certification required
- Current ACLS certification required
- Documented med/tele experience is required and must be supported by checklist
- Familiarity with EPIC EMR for documentation and charting
- Experience with telemetry monitoring, medication administration, and acute care interventions
- Prior skilled nursing, rehabilitation, or NICHE training is a strong plus
- Ability to upload all demographic information and provide a badge-appropriate photo as requested
Knowledge & Skills
- Telemetry monitoring and cardiac rhythm recognition
- Medication administration including IV drips and titratable infusions
- Acute patient assessment and bedside clinical decision making
- Proficient documentation in EPIC EMR
- Safe patient handling using gait belts and mobility aids
- Familiarity with trauma care basics and working with older adult populations
- Skilled nursing and rehabilitation support practices
- Clear communication and teamwork in fast-paced acute care settings
