Roles & Responsibilities

• Provide skilled occupational therapy evaluation and treatment to adult patients across Livingston and Washtenaw Counties
• Design and implement meaningful, individualized treatment plans that improve daily living and functional independence
• Collaborate with interdisciplinary teams, families and caregivers to coordinate care and support transitions
• Document thoroughly and accurately in EPIC to ensure continuity of care and compliance
• Participate in Trinity specialty competency testing and skills checklists to maintain clinical excellence
• Educate patients and families about strategies, adaptive equipment and home safety to support long-term outcomes

Qualifications

• Valid state Occupational Therapy license (license verification required)
• BLS certification required
• Experience with EPIC strongly preferred
• Completion of Trinity specialty specific competency testing and Trinity skills checklist as part of onboarding
• Current immunizations and health screening: COVID-19 vaccine, MMR, Hepatitis B, Tdap, influenza, Varicella, 2-step PPD or IGRA, and a recent physical/health statement
• Successful completion of Trinity 10 panel drug screen and fit test

Education

• Degree in Occupational Therapy (OTR/OT) or equivalent from an accredited program
• State licensure as an Occupational Therapist

Knowledge & Skills

• Proficient use of EPIC for documentation and care coordination
• Strong assessment skills for ADL/IADL, transfers and mobility
• Skilled in developing patient-centered treatment plans and adaptive equipment recommendations
• Excellent communication and teamwork skills for working with multidisciplinary teams
• Familiarity with rehab outcome measurement and progress reporting
