Roles & Responsibilities

• Evaluate and document patient function and progress using evidence-based assessment tools
• Design and implement individualized treatment plans that optimize mobility and independence
• Provide hands-on interventions and therapeutic exercises that promote meaningful functional gains
• Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care
• Educate patients and families on home exercise programs and strategies to maintain progress
• Maintain timely, accurate clinical documentation and communicate effectively with referral sources

Qualifications

• Current Minnesota Physical Therapist license or ability to obtain licensure in MN
• Doctor of Physical Therapy (DPT) preferred, or equivalent degree in Physical Therapy
• Basic Life Support (BLS) or CPR certification (or ability to obtain)
• Outpatient orthopedics or general outpatient experience preferred
• Strong communication skills and a collaborative, patient-first approach
• Ability to pass standard background and credentialing checks

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree

Knowledge & Skills

• Proficient in musculoskeletal evaluation and therapeutic exercise prescription
• Skilled in manual therapy techniques and functional mobility training
• Comfortable with outcome measures and progress tracking
• Competent documentation skills and experience with electronic medical records
• Excellent interpersonal skills for patient education and team collaboration
