Shift Type: Rotating day/evening shifts: Monday-Friday (starts at 6:30 AM or 8:00 AM, rotating start times) with rotating weekends 7:00 AM - 12:00 PM; 40 hours per week (rotating schedule)

Candidate Pay Rate: $84 per hour

Roles & Responsibilities
- Prepare and accurately dispense medication orders, ensuring safe and timely therapy for patients
- Compound sterile preparations including IV admixtures, chemotherapy, and parenteral nutrition with strict adherence to protocols
- Maintain medication stock in patient care areas and manage controlled substances in compliance with law
- Monitor drug therapy regimens for interactions, contraindications, allergies, and dosing appropriateness
- Perform pharmacokinetic dosing and therapeutic drug monitoring per hospital protocols
- Review culture & sensitivity (C&S) data and recommend targeted antibiotic therapy when appropriate
- Provide clinical pharmacy consultations and drug information to practitioners, patients, and families
- Document clinical activities and interventions clearly and participate in quality improvement and medication review initiatives
- Serve as an active member of the Code Response Team when clinical emergencies arise
Qualifications
- Active CO PHA (Pharmacist) license in good standing — required
- BS Pharmacy from an ACPE-accredited School of Pharmacy — required
- Epic experience is required; Meditech training is a plus (facility is converting from Meditech to Epic)
- BLS (American Heart Association) — required
- ACLS (American Heart Association) — required
- Proof of flu vaccine required (no exemptions) for Oct 1 through April 1
- Strong sterile compounding and IV admixture experience
- Comfortable working rotating day/evening shifts and rotating weekends
Education
- BS Pharmacy (ACPE-accredited School of Pharmacy)
Knowledge & Skills
- Medication dispensing and clinical pharmacy consultation
- IV admixture and sterile compounding (including chemotherapy and parenteral nutrition)
- Pharmacokinetic dosing and therapeutic drug monitoring
- Controlled substances handling and regulatory compliance
- Formulary management and therapeutic substitution
- EMR systems: Epic (required) and Meditech (advantageous)
- C&S interpretation and antimicrobial stewardship principles
- Medication safety, quality improvement, and clear clinical documentation
- Effective patient and provider education and communication
