Roles & Responsibilities

• Perform comprehensive PT evaluations and fall risk assessments to establish individualized plans of care
• Design and deliver evidence-based orthopedic rehabilitation programs, including post-op care for total joint and rotator cuff repairs
• Manage and grow an active caseload (typical target ~25 active patients, 8-10 visits/day) while maintaining high-quality care
• Document thoroughly and efficiently in NextGen EHR to support continuity of care and billing
• Coordinate care and communicate effectively with patients, families, and interdisciplinary team members
• Screen and build caseloads through referrals and active patient outreach to maximize clinic productivity and impact

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy; new graduates are welcome to apply
• Licensed or eligible for Physical Therapist licensure in Michigan
• Preferred: 1+ year of outpatient orthopedic or outpatient PT experience (new grads considered at a reduced bill rate)
• Experience with orthopedic conditions including total joint replacement and rotator cuff repair is a strong plus
• Comfortable using NextGen EHR or similar electronic health record systems
• Strong case management, screening, and caseload-building skills
• Excellent communication skills and a professional, business-casual approach

Education
