Roles & Responsibilities

• Provide compassionate, evidence-based physical therapy evaluations and skilled interventions in patients' homes
• Develop individualized treatment plans focused on functional gains, safety, and independence
• Deliver hands-on skilled care including gait training, balance work, therapeutic exercise, and adaptive techniques
• Coordinate care with physicians, nurses, and family members to ensure continuity and quality
• Document timely, thorough clinical notes and outcome measures following agency and regulatory standards
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility strategies
• Participate in required competency testing and skills checklists to maintain high-quality practice

Qualifications

• Current Physical Therapist license (state of practice) — license verification required
• BLS (Basic Life Support) certification
• Home health experience or strong interest in community-based care preferred
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as required
• Ability to pass required health screenings and background checks (drug screen, immunizations, TB testing)
• Epic experience preferred but not required

Knowledge & Skills

• Skilled in home health assessment, treatment planning, and functional outcome measurement
• Proficient with patient education, safe transfer training, and adaptive equipment recommendations
• Strong clinical reasoning and time-management to manage a mobile caseload
• Excellent communication and collaboration with multidisciplinary teams and caregivers
• Comfortable with documentation systems; Epic experience is a plus
• Ability to adapt interventions to varied home environments and patient needs
