Roles & Responsibilities
- Provide skilled physical therapy interventions and carry out treatment plans designed to restore mobility and function
- Assist with gait training, therapeutic exercise and use of adaptive equipment to promote resident independence
- Document patient progress accurately and in a timely manner to support continuity of care
- Collaborate with nurses, therapists and interdisciplinary team members to create meaningful rehab goals
- Educate residents and families on home programs, safety and fall prevention to support long-term outcomes
- Maintain a safe, compassionate environment while adhering to facility policies and regulatory standards
Qualifications
- PTA credential or completion of an accredited PTA program preferred; open to new grads as noted in the order
- Comfort working in a skilled nursing facility environment
- Strong communication skills and a team-oriented mindset
- Current basic life support (BLS) as required by facility policies (obtainable if needed)
- Ability to pass required background checks and facility onboarding
Education
- Graduation from an accredited Physical Therapist Assistant program preferred
Knowledge & Skills
- Patient assessment and basic mobility evaluation skills
- Hands-on therapeutic modalities, exercise prescription and gait training
- Skilled documentation and familiarity with clinical charting
- Knowledge of resident safety, fall prevention and safe transfers
- Good interpersonal skills for working with residents, families and the care team
