Roles & Responsibilities

• Verify and prepare oncology infusion medications with strict attention to safety
• Perform clinical dosing and pharmacokinetic dosing for complex therapies
• Provide therapeutic medication monitoring and dose adjustments
• Prepare sterile IV products including IV piggyback and IV push per protocol
• Document accurately in Epic and ensure timely charting of medication administration
• Collaborate with oncologists, nurses, and the care team to optimize therapy plans
• Educate patients and caregivers on medication use, side effect management, and safety
• Maintain compliance with state pharmacy regulations and facility policies
• Support compounding and use of medication dispensing systems including Baxter

Qualifications

• PharmD (Doctor of Pharmacy) — required
• Licensed Pharmacist in California (Pharmacist State of CA) — required
• Active RPh license or equivalent credential
• Minimum 2 years recent Oncology clinical infusion experience — required
• Epic experience — required (charting and medication management)
• BLS certification preferred
• Comfort with sterile compounding, IV preparation, and medication dispensing systems
• Ability to float as needed and walk between locations

Education

• PharmD (Doctor of Pharmacy) required
