Roles & Responsibilities
- Conduct comprehensive home assessments and safety evaluations to craft individualized interventions
- Perform evidence-based patient assessments and document OASIS encounters accurately in EPIC
- Develop, implement, and revise patient-centered Plans of Care that prioritize independence and safety
- Provide patient and family education on adaptive equipment, transfers, and home strategies
- Recommend, fit, and train patients on adaptive equipment and mobility aids
- Manage a focused caseload (may visit up to 6 patients/day) while maintaining high-quality documentation and productivity
- Coordinate care with interdisciplinary team members and community resources to support discharge planning and community re-entry
- Use Rover mobile tools for scheduling and visit documentation while minimizing drive time through efficient routing
Qualifications
- Current Oregon Occupational Therapy license (OR license) — required
- Minimum 1 year of skilled Home Health experience
- Basic Cardiac Life Support (BCLS/BLS) certification — required
- Experience with OASIS documentation — please highlight if you have it (OASIS required)
- Familiarity with EPIC charting and Rover mobile app preferred
- Own reliable transportation and willingness to travel within a roughly 60-mile daily radius
- First-time travel clinicians are welcome — enthusiastic first-timers accepted
Education
- Degree in Occupational Therapy (OT, MOT, or OTD) — entry-level OT degree required
- Current state licensure for Occupational Therapy (Oregon)
Knowledge & Skills
- Strong home health clinical skills: patient assessment, plan of care, reassessments, and discharge planning
- Proficient documentation practices including OASIS and EPIC Remote charting
- Skilled in adaptive equipment recommendations, wheelchair evaluation and positioning
- Comfortable with orthopedic and neurorehabilitation diagnoses (post-op ortho, CVA, Parkinson’s, ALS, MS)
- Hands-on skills: joint mobilization, myofascial release, splinting, prosthetics/orthotics exposure
- Excellent patient and family education, safety and injury prevention strategies
- Good time management and route planning to maximize visits and minimize windshield time
- Familiarity with National Patient Safety Goals and billing/documentation workflows
