Contract Type: Travel Contract
Shift Type: Monday - Friday, Day shift, 5x8-hour (possible times: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM); weekends required every other weekend (or 4 weekend days in a 4-week schedule)
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Coordinate and manage patient care across the hospital to ensure safe, timely discharges
- Perform utilization review and apply Milliman Guidelines to support appropriate resource use
- Develop and implement discharge plans and plans of care in collaboration with the interdisciplinary team
- Document thoroughly in Epic and maintain compliance with HIPAA and CMS regulatory expectations
- Conduct needs assessments and order durable medical equipment as needed
- Provide patient and family education to support transitions of care and reduce readmissions
- Float within case management services as needed to cover gaps and support unit needs
Qualifications
- Active AK RN license in hand required for auto offer (screen for pending license; AK RN license required)
- Minimum 1 year acute care experience required; first-time travel clinicians accepted
- BLS required
- ACM (Case Management) certification preferred
- Epic experience required (Epic hardstop requirement; negotiable with the right candidate)
- Ability to meet weekend rotation requirements and float within scope
- Locals not accepted: permanent address must be at least 50 miles from facility
Education
- Registered Nurse education required as per AK RN licensure (ADN or BSN acceptable)
Knowledge & Skills
- Strong Epic charting skills and experience with acute hospital workflows
- Knowledge of Milliman Guidelines and utilization review practices
- Care coordination, discharge planning, and case management expertise
- Familiarity with CMS regulations and HIPAA guidelines
- Comfortable using Microsoft Office and Teams for communication and documentation
- Ability to work with diverse patient age groups from neonates to geriatrics
