Shift Type: Monday - Friday, Day shift, 5x8-hour (possible times: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM); weekends required every other weekend (or 4 weekend days in a 4-week schedule)

Roles & Responsibilities

• Coordinate and manage patient care across the hospital to ensure safe, timely discharges
• Perform utilization review and apply Milliman Guidelines to support appropriate resource use
• Develop and implement discharge plans and plans of care in collaboration with the interdisciplinary team
• Document thoroughly in Epic and maintain compliance with HIPAA and CMS regulatory expectations
• Conduct needs assessments and order durable medical equipment as needed
• Provide patient and family education to support transitions of care and reduce readmissions
• Float within case management services as needed to cover gaps and support unit needs

Qualifications

• Active AK RN license in hand required for auto offer (screen for pending license; AK RN license required)
• Minimum 1 year acute care experience required; first-time travel clinicians accepted
• BLS required
• ACM (Case Management) certification preferred
• Epic experience required (Epic hardstop requirement; negotiable with the right candidate)
• Ability to meet weekend rotation requirements and float within scope
• Locals not accepted: permanent address must be at least 50 miles from facility

Education

• Registered Nurse education required as per AK RN licensure (ADN or BSN acceptable)

Knowledge & Skills

• Strong Epic charting skills and experience with acute hospital workflows
• Knowledge of Milliman Guidelines and utilization review practices
• Care coordination, discharge planning, and case management expertise
• Familiarity with CMS regulations and HIPAA guidelines
• Comfortable using Microsoft Office and Teams for communication and documentation
• Ability to work with diverse patient age groups from neonates to geriatrics
