Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:30 AM - 4:00 PM Day (5x8 hours). Primary day shift is 7:30 AM - 4:00 PM; other possible scheduled day shifts include 7:00 AM - 3:30 PM, 8:00 AM - 4:30 PM, and 8:30 AM - 5:00 PM. Weekends required: every other weekend or 4 weekend days in a 4-week schedule. Day shift, with occasional floating within case management services to meet facility needs.

Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Coordinate patient discharge planning to ensure safe, timely transitions from hospital to home or next level of care
- Conduct utilization review using Milliman Guidelines to support appropriate level-of-care decisions
- Perform needs assessments and order durable medical equipment (DME) as part of individualized plan of care
- Document comprehensive case management activities and outcomes in Epic with accuracy and timeliness
- Collaborate with interdisciplinary teams (therapy, pharmacy, social work, interpretation services) to remove barriers to care
- Provide care coordination across age groups from neonates to geriatrics, tailoring plans to each patient’s needs
- Float within case management services as needed to cover gaps and support department workflow
Qualifications
- AK RN license in hand required for auto-offer (screen for pending license as instructed)
- Minimum of 1 year acute care experience (first timers accepted)
- Epic EMR experience required (Epic hardstop requirement; negotiable for exceptional candidates)
- BLS certification required; ACM (case management) certification preferred
- Experience with utilization review, discharge planning, care coordination, and plan-of-care development
- Willingness to work required weekend rotation (every other weekend or 4 weekend days per 4-week schedule)
- Must be able to float within case management scope to meet facility needs; locals not accepted (permanent address must be at least 50 miles from facility)
Knowledge & Skills
- Proficiency with Epic charting and workflows
- Knowledge of Milliman Guidelines for utilization review
- Strong care coordination and discharge planning skills
- Ability to perform needs assessments and order DME
- Familiarity with CMS regulations and HIPAA compliance
- Comfort working across patient populations: neonates through geriatrics
- Basic computer skills including Microsoft Office and Teams (preferred)
- Effective communication and interdisciplinary collaboration skills
