Roles & Responsibilities
- Provide skilled physical therapy assistance to patients in their homes, focusing on functional improvement and safety
- Implement and progress treatment plans developed by the supervising PT, delivering purposeful, patient-centered care
- Conduct thorough functional mobility and safety assessments and recommend adaptive equipment or home modifications
- Document visits accurately and efficiently using EPIC Remote, OASIS and Rover to ensure continuity of care
- Coordinate care with nurses, social workers and other members of the interdisciplinary team
- Educate patients and caregivers on exercise programs, fall prevention strategies and safe transfer techniques
- Manage a caseload typically up to six visits per day while meeting productivity expectations
- Maintain BCLS certification and follow infection control and safety protocols while traveling between patient homes
Qualifications
- Current Physical Therapist Assistant (PTA) credential and ability to hold OR or WA licensure (pending licenses may be accepted; confirm with recruiter)
- Minimum 1 year of skilled home health experience required
- BCLS (Basic Cardiac Life Support) certification required
- Reliable transportation and a valid driver’s license — traveler expected to have their own vehicle
- Comfortable using EPIC, OASIS and Rover for charting and clinical documentation
- Able to travel up to roughly 60 miles (occasionally longer) in a day and manage drive time between visits
- First-time travel clinicians are welcome to apply
Education
- Graduate of an accredited Physical Therapist Assistant program
- Physical Therapist Assistant credential and state licensure (PTA)
Knowledge & Skills
- Skilled in home health assessment, treatment planning and documentation
- Proficient with gait training, transfers, balance and therapeutic exercise techniques
- Experience recommending and training on adaptive equipment and home safety solutions
- Strong documentation skills in EPIC, OASIS and mobile apps like Rover
- Excellent time management, route planning and caseload organization
- Clear, compassionate communication with patients, families and interdisciplinary teams
- Ability to work independently and problem-solve in varied home environments
