Roles & Responsibilities
- Provide hands-on therapeutic treatment and assist patients with mobility, strength, and functional goals
- Deliver quality care within the skilled nursing and assisted living setting, following the established plan of care
- Collaborate closely with the therapy team and onsite Director of Rehab to coordinate patient progress
- Document patient interventions and progress accurately and timely to meet facility and CRG policies
- Maintain required productivity standards (90%) while ensuring individualized, compassionate care
- Adhere to facility protocols including dress code (black scrubs) and CRG policies & procedures
Qualifications
- PTA (Physical Therapist Assistant) experience and skills as listed in the job order
- 1-3 years of relevant experience working in therapy or rehabilitation settings (SNF experience preferred)
- Comfortable working in a 160-bed skilled nursing and assisted living facility environment
- Ability to be onsite and clocked in to receive guaranteed hours
- Willingness to follow CRG policies and meet a 90% productivity requirement
Knowledge & Skills
- Patient assessment and ability to implement therapeutic interventions
- Knowledge of common SNF diagnoses and appropriate rehab strategies
- Strong documentation skills and familiarity with clinical recordkeeping
- Teamwork and communication with interdisciplinary staff
- Time management to balance productivity expectations with quality care
