Contract Type: Travel Contract
Shift Type: Monday-Friday, Day shift (one of the following start/end times: 7:00 AM - 3:30 PM; 7:30 AM - 4:00 PM; 8:00 AM - 4:30 PM; 8:30 AM - 5:00 PM). Weekend coverage required: every other weekend or 4 weekend days in a 4-week schedule. Float within case management services as needed.
Hours per Week: 40
Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Coordinate patient care and lead discharge planning to ensure safe, timely transitions
- Perform utilization review and case management activities using Milliman guidelines
- Document accurately in Epic and meet CMS and HIPAA regulatory standards
- Assess needs, arrange DME orders, and develop individualized plans of care
- Collaborate with interdisciplinary teams including social services, pharmacy, PT, and respiratory
- Support department staffing gaps by floating within case management scope
- Communicate clearly with patients, families, and clinical teams to advocate for patient-centered care
Qualifications
- AK RN license in hand required for offer (pending license screened but hand license required for auto-offer)
- Minimum of 1 year acute care nursing experience
- Epic EMR experience required
- BLS certification required
- ACM certification preferred
- Comfortable with weekend coverage and occasional floating to meet facility needs
- Locals not accepted: candidate permanent address must be at least 50 miles from the facility
Knowledge & Skills
- Strong knowledge of case management and utilization review processes
- Familiarity with Milliman guidelines for utilization and discharge decisions
- Proficient Epic charting skills, including Epic hardstop workflows
- Care coordination, discharge planning, and needs assessment expertise
- Comfortable using Microsoft Office and Teams for communication and documentation
- Understanding of CMS regulations and HIPAA requirements
- Excellent communication, organization, and interdisciplinary collaboration skills
