Roles & Responsibilities

• Provide hands-on skilled physical therapy interventions to help patients regain function and independence
• Assess patient mobility, strength, balance and functional needs, then develop and implement treatment plans
• Document patient progress clearly and timely in the facility’s EMR to support continuity of care
• Collaborate with PTs, nursing staff and interdisciplinary team members to coordinate care goals
• Provide patient and family education to promote safe transfers, home exercise programs and fall prevention
• Assist with safe use and maintenance of therapy equipment and adaptive devices
• Advocate for patient-centered goals and contribute to positive outcomes in the SNF setting

Qualifications

• Current or eligible for Virginia PTA licensure (state licensure required prior to start)
• Graduation from an accredited Physical Therapist Assistant program
• Basic Life Support (BLS/CPR) certification or ability to obtain upon hire
• Open to any experience level — new grads welcome and encouraged to apply
• Strong communication, clinical reasoning and documentation skills
• Ability to pass background check and meet facility onboarding requirements

Education

• Associate degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Skilled in therapeutic exercise, transfers, gait training and mobility assistance
• Knowledge of common SNF diagnoses, restorative nursing and long-term care rehab
• Familiarity with documentation best practices and EMR charting
• Safe patient handling techniques and use of assistive devices
• Time management and prioritization in a busy rehab environment
• Teamwork and the ability to collaborate across disciplines
