Roles & Responsibilities
- Provide hands-on skilled physical therapy interventions to help patients regain function and independence
- Assess patient mobility, strength, balance and functional needs, then develop and implement treatment plans
- Document patient progress clearly and timely in the facility’s EMR to support continuity of care
- Collaborate with PTs, nursing staff and interdisciplinary team members to coordinate care goals
- Provide patient and family education to promote safe transfers, home exercise programs and fall prevention
- Assist with safe use and maintenance of therapy equipment and adaptive devices
- Advocate for patient-centered goals and contribute to positive outcomes in the SNF setting
Qualifications
- Current or eligible for Virginia PTA licensure (state licensure required prior to start)
- Graduation from an accredited Physical Therapist Assistant program
- Basic Life Support (BLS/CPR) certification or ability to obtain upon hire
- Open to any experience level — new grads welcome and encouraged to apply
- Strong communication, clinical reasoning and documentation skills
- Ability to pass background check and meet facility onboarding requirements
Education
- Associate degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Skilled in therapeutic exercise, transfers, gait training and mobility assistance
- Knowledge of common SNF diagnoses, restorative nursing and long-term care rehab
- Familiarity with documentation best practices and EMR charting
- Safe patient handling techniques and use of assistive devices
- Time management and prioritization in a busy rehab environment
- Teamwork and the ability to collaborate across disciplines
