Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day); Every-other-weekend coverage required
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Coordinate comprehensive pediatric case management to ensure smooth transitions of care
- Lead discharge planning efforts, collaborating with families and multidisciplinary teams
- Assess patient needs across NICU, PICU and pediatric populations and develop effective care plans
- Communicate with clinicians, specialists and community resources to optimize care coordination
- Document thoroughly in the required charting system to maintain accurate patient records
- Float to other UCSF facilities as needed to support staffing and continuity of care
- Support patient and family education, empowering caregivers for safe discharges
Qualifications
- CA RN License (required)
- AHA BLS (required)
- Minimum 2 years pediatric case management experience, including discharge planning
- Experience in NICU and/or PICU settings
- Trauma Level II experience (required); Trauma Level I experience is a plus
- Travel experience required and comfortable with floating between facilities
- Proven ability to manage higher patient ratios and prioritize effectively
- Charting system experience (required)
Education
- Registered Nurse licensure (RN) required
- Master's in Nursing or healthcare-related field preferred
Knowledge & Skills
- Strong knowledge of pediatric case management and care coordination
- Familiarity with NICU and PICU clinical environments and common neonatal/pediatric diagnoses
- Excellent communication and interdisciplinary collaboration skills
- Skilled in discharge planning, utilization review, and community resource linkage
- Comfortable with electronic charting systems and accurate clinical documentation
- Ability to adapt to changing unit needs and float between facilities
