Contract Type: Travel Contract
Shift Type: Monday-Friday (5x8 Day shifts), 7:30 AM - 4:00 PM Day (actual start/end may vary; possible schedules include 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM). Weekends required every other weekend (or 4 weekend days in a 4-week schedule).

Candidate Pay Rate: $71 per hour

Roles & Responsibilities
- Coordinate safe, timely discharges and build clear plans of care with patients, families, and multidisciplinary teams
- Conduct utilization review and apply Milliman Guidelines to support appropriate level of care
- Perform needs assessments and order durable medical equipment to support post-discharge needs
- Document thoroughly and accurately in Epic, including adherence to Epic hardstop requirements
- Facilitate care coordination across inpatient teams, outpatient providers, and community resources
- Assist with workflow coverage by floating within case management services where gaps exist
- Support regulatory compliance such as CMS and HIPAA through meticulous documentation and best practices
Qualifications
- AK RN license in hand required for Auto Offer (screen for pending license but license in hand preferred)
- Minimum 1 year acute care nursing experience required
- BLS certification required
- ACM certification preferred and encouraged
- Epic experience required (Epic hardstop familiarity preferred)
- Comfort with utilization review, discharge planning, and care coordination responsibilities
- Willingness to work every other weekend or meet 4 weekend days in a 4-week schedule
- Ability to float within scope to meet facility needs
- Candidates must have a permanent address at least 50 miles from the facility (locals not accepted)
Knowledge & Skills
- Case management and utilization review best practices
- Discharge planning and developing individualized plans of care
- Epic EMR proficiency, including hardstop workflows
- Milliman Guidelines application for appropriateness of care
- Needs assessment and ordering DME
- Strong interdisciplinary communication and care coordination
- Familiarity with acute hospital workflows and regulatory expectations (CMS, HIPAA)
- Comfortable using Microsoft Office and Teams for communication and documentation
