Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:30 AM - 4:00 PM Day (5x8) with every other weekend required; floats between Colby and Pacific campuses as needed
Hours per Week: 40
Candidate Pay Rate: $52 per hour

Roles & Responsibilities
- Conduct comprehensive biopsychosocial assessments and initial evaluations to guide patient care
- Coordinate discharge planning and collaborative care with inpatient teams, community providers, and families
- Arrange home care services, durable medical equipment orders, and referrals to rehab or long-term care
- Document care delivery, plans, goals and communications clearly in EPIC to ensure continuity
- Navigate Medicaid, Medicare and community resources to connect patients with financial and transportation assistance
- Manage adult protective services, guardianship and power of attorney issues with sensitivity and compliance
- Provide care across diverse settings including medical-surgical, ED, ICU, L&D/Mother-Baby and NICU when needed
Qualifications
- Master’s in Social Work (MSW) preferred; LMSW or LCSW candidates accepted per facility guidance
- Washington clinical or associate social work license accepted (LICSW, LSWAIC, LASW, LSWAA) — pending license accepted at submittal, but clinician must have Washington license in hand as required by facility
- Minimum 1 year of experience in acute care with discharge planning and care coordination experience
- BLS certification required
- Epic charting experience required
- Ability to float across hospital units and cover varied patient populations, including adult and geriatric caseloads
Education
- Master’s in Social Work (MSW) preferred; facility accepts LMSW and LCSW credentialed clinicians
Knowledge & Skills
- Biopsychosocial assessment and initial evaluation expertise
- Strong discharge planning, care coordination and case management skills
- Knowledge of Medicaid, Medicare, medical transportation and medication assistance programs
- Experience with guardianship, power of attorney and adult protective services
- Comfort working in fast-paced acute care settings including ED, ICU, Med-Surg, L&D and NICU
- Excellent documentation and communication skills within EPIC
- Cultural competence and experience supporting patients with communication or vulnerability concerns
