Roles & Responsibilities

• Coordinate comprehensive case management services across the continuum of care to ensure safe, timely transitions
• Develop and maintain individualized plans of care that reflect patient needs, goals, and regulatory requirements
• Conduct utilization review and apply Milliman guidelines to support appropriate level-of-care decisions
• Lead discharge planning efforts and arrange post-acute resources, DME, and community supports
• Document thoroughly and accurately in Epic to support continuity of care and billing/regulatory compliance
• Collaborate with interdisciplinary teams — physicians, social work, pharmacy, therapy, and others — to remove barriers to discharge
• Communicate with patients and families with empathy and clarity to promote understanding and engagement
• Float within case management services as needed to cover gaps while staying within your scope of practice

Qualifications

• AK RN license in hand required for auto-offer (screen for pending license; pending may be considered per guidelines)
• Current BLS certification required
• ACM (Acute Care Case Management) certification preferred
• Minimum 1 year of acute care experience required; Epic EMR experience required
• Comfort working Monday–Friday business hours and providing weekend coverage per department needs
• First-time travel clinicians are welcome
• Must be located at least 50 miles from the facility (locals not accepted for this assignment)

Knowledge & Skills

• Strong case management and care coordination skills
• Epic charting and documentation expertise
• Experience with discharge planning and needs assessment / ordering DME
• Utilization review knowledge and familiarity with Milliman Guidelines
• Ability to develop and manage plans of care
• Proficiency with Microsoft Office and Teams (preferred)
• Clinical familiarity with Pediatrics, Women’s Services, ICU, SDU/PCU/IMC/Obs settings (preferred)
• Understanding of CMS and HIPAA regulations
