Roles & Responsibilities

• Provide excellent patient-centered physical therapy care across inpatient rehabilitation, swing bed rehabilitation, and outpatient orthopedics
• Perform thorough assessments and develop individualized treatment plans to help patients regain function and independence
• Deliver skilled orthopedic and, as appropriate, neurological rehabilitation interventions based on patient needs and therapist experience
• Collaborate closely with nurses, physicians, case managers, and the interdisciplinary team to coordinate care and achieve patient goals
• Document skilled services clearly and timely in the medical record and participate in Trinity Specialty Specific Competency Testing and Skills Checklists
• Support discharge planning and patient/family education to ensure safe transitions and ongoing recovery

Qualifications

• Current, unrestricted Physical Therapy license (state license verification required)
• BLS certification required
• Experience or comfort working in inpatient rehabilitation, swing bed settings, and outpatient orthopedics; neurological rehab experience is a plus
• Willingness to complete Trinity competency testing and skills checklist
• Ability to pass required health screenings and background checks, including drug screen and immunizations
• Strong communication skills and ability to work collaboratively in a rural hospital setting

Knowledge & Skills

• Skilled in orthopedic assessment and intervention techniques
• Familiarity with inpatient and swing bed rehabilitation protocols
• Knowledge of neurological rehabilitation principles preferred
• Effective documentation and clinical reasoning skills
• Excellent communication and patient education abilities
• Time management and ability to manage a varied caseload
