Roles & Responsibilities
- Evaluate patients and develop individualized treatment plans focused on restoring mobility and independence
- Deliver evidence-based therapeutic interventions such as therapeutic exercise, gait training, and balance work
- Collaborate with nursing and interdisciplinary teams to coordinate safe, patient-centered care
- Document progress and outcomes clearly and timely to support continuity of care
- Educate patients and families on home programs and safe functional strategies
- Participate in discharge planning and make appropriate referrals to ensure optimal transitions
Qualifications
- Doctor of Physical Therapy (DPT) preferred — new grads welcome
- Current Illinois Physical Therapist license or eligible to obtain licensure
- Experience or interest in skilled nursing facility (SNF) care
- Strong communication and teamwork skills to collaborate with interdisciplinary staff
- Basic life support (BLS) preferred or willingness to obtain
Education
- Doctor of Physical Therapy (DPT) preferred; entry-level PT credentials accepted
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise, gait training, and transfers
- Familiarity with documentation standards and SNF regulatory expectations
- Ability to design safe, progressive home and facility-based programs
- Good clinical reasoning and problem-solving in a fast-paced setting
- Patient- and family-centered education and motivational coaching
