Roles & Responsibilities

• Provide high-quality physical therapy interventions and assist patients with mobility, transfers, and therapeutic exercise
• Collaborate closely with the rehab team and the on-site Director of Rehab to deliver coordinated care
• Document patient progress and treatment plans accurately using NetHealth and PCC
• Meet department productivity expectations (90%) while maintaining patient-centered care
• Adhere to Continuum Rehabilitation Group policies and facility dress code (black scrubs) and be present/clocked in for guaranteed hours
• Communicate effectively with patients, families, and interdisciplinary staff to support optimal outcomes

Qualifications

• Licensed/credentialed Physical Therapist Assistant (PTA) as required by Missouri (verify state requirements)
• 1-3 years of experience working in a skilled nursing facility or similar rehab setting preferred
• Familiarity with NetHealth and PCC documentation systems (experience is a plus)
• Ability to meet productivity expectations and follow CRG policies and procedures
• Reliable, on-site presence required (therapist must be present and clocked-in to receive guaranteed hours)
• Professional appearance and adherence to dress code (black scrubs)

Knowledge & Skills

• Skilled in therapeutic exercise, ambulation training, transfers, and fall prevention strategies
• Strong documentation skills, comfortable using NetHealth and PCC
• Team-oriented approach and clear communication with nursing and rehab staff
• Time management and productivity focus within a busy SNF environment
• Patient assessment and safety-first mindset
