Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:00 AM - 5:00 PM (actual shift varies; possible times include 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, 8:30 AM - 5:00 PM); Day shift, 5x8. Weekends required every other weekend or 4 weekend days in a 4-week schedule. Floating within case management services may be required.
Hours per Week: 40
Candidate Pay Rate: $34 per hour

Roles & Responsibilities
- Coordinate patient care across the continuum by developing and managing individualized plans of care
- Conduct utilization review and apply Milliman Guidelines to support appropriate level of care decisions
- Lead discharge planning activities and collaborate with interdisciplinary teams to ensure safe transitions
- Assess needs and order durable medical equipment and community resources to support post-discharge success
- Document thoroughly in Epic to maintain accurate, timely case records and support regulatory requirements
- Communicate proactively with patients, families, and community partners to optimize outcomes and reduce readmissions
- Float within the case management service as needed to fill coverage gaps and maintain continuity of care
Qualifications
- AK RN license in hand required for auto-offer (screen for pending license; must be active)
- BLS certification required
- Minimum 1 year of acute care hospital experience as an RN
- Epic experience required (experience with inpatient charting systems)
- Case management or utilization review experience strongly preferred
- ACM certification preferred but not required
- Willingness to work required weekend schedule and to float within case management services
- Permanent address must be at least 50 miles from the facility (locals not accepted for this opening)
Knowledge & Skills
- In-depth knowledge of case management principles, discharge planning, and care coordination
- Familiarity with Milliman Guidelines for utilization review
- Proficient with Epic electronic health record workflows and documentation
- Comfortable navigating interdisciplinary teams including social services, pharmacy, and therapy services
- Basic computer skills including Microsoft Office and Teams for communication and documentation
- Strong clinical assessment, communication, and patient advocacy skills
