Roles & Responsibilities

• Perform comprehensive OASIS assessments during home visits to capture accurate clinical status
• Provide skilled nursing and hospice support to patients in their homes with compassion and professionalism
• Document all patient care thoroughly and accurately in the HealthWyse (Careficient) EMR
• Coordinate care with interdisciplinary teams, families and community resources to ensure continuity
• Administer medications, wound care and other skilled nursing treatments as ordered
• Educate patients and caregivers on disease management, safety and care plans
• Manage and prioritize your caseload while safely traveling between patient homes
• Respond to emergency on-call situations when they arise

Qualifications

• Current LPN license in good standing
• Home Health and Hospice experience required with demonstrated OASIS experience
• BLS certification required
• Valid driver's license and reliable transportation for home visits
• Comfortable using HealthWyse (Careficient) EMR for documentation
• Ability to adapt to schedule changes and handle travel between patient homes

Education

• Graduate of an accredited LPN program or equivalent

Knowledge & Skills

• Strong knowledge of OASIS assessments and home health documentation requirements
• Proficiency with EMR documentation, preferably HealthWyse or Careficient
• Skilled clinical assessment, medication administration and wound care
• Excellent communication and patient education skills
• Time management and route planning for efficient home visit scheduling
• Compassionate hospice care practices and end of life support
