Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day); occasional weekend coverage and emergency on-call as needed

Candidate Pay Rate: $43 per hour

Roles & Responsibilities
- Perform comprehensive OASIS assessments during home visits to capture accurate clinical status
- Provide skilled nursing and hospice support to patients in their homes with compassion and professionalism
- Document all patient care thoroughly and accurately in the HealthWyse (Careficient) EMR
- Coordinate care with interdisciplinary teams, families and community resources to ensure continuity
- Administer medications, wound care and other skilled nursing treatments as ordered
- Educate patients and caregivers on disease management, safety and care plans
- Manage and prioritize your caseload while safely traveling between patient homes
- Respond to emergency on-call situations when they arise
Qualifications
- Current LPN license in good standing
- Home Health and Hospice experience required with demonstrated OASIS experience
- BLS certification required
- Valid driver's license and reliable transportation for home visits
- Comfortable using HealthWyse (Careficient) EMR for documentation
- Ability to adapt to schedule changes and handle travel between patient homes
Education
- Graduate of an accredited LPN program or equivalent
Knowledge & Skills
- Strong knowledge of OASIS assessments and home health documentation requirements
- Proficiency with EMR documentation, preferably HealthWyse or Careficient
- Skilled clinical assessment, medication administration and wound care
- Excellent communication and patient education skills
- Time management and route planning for efficient home visit scheduling
- Compassionate hospice care practices and end of life support
