Roles & Responsibilities

• Evaluate and assess patients to develop individualized treatment plans that restore mobility and independence
• Implement therapeutic interventions to improve strength, balance, coordination, and overall function
• Monitor patient progress and adjust treatment plans to achieve the best outcomes
• Educate patients and caregivers on home exercises, safety strategies, and injury prevention
• Maintain accurate, timely documentation in line with facility and state guidelines
• Collaborate with interdisciplinary team members to deliver comprehensive, patient-centered care

Qualifications

• Active California Physical Therapist (PT) license
• Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy (DPT) or equivalent)
• Current BLS/CPR certification
• Minimum 1 year of clinical experience preferred; new graduates may be considered depending on the setting
• Strong communication and organizational skills with a patient-focused mindset

Education

• Doctor of Physical Therapy (DPT) or equivalent from an accredited Physical Therapy program

Knowledge & Skills

• Patient assessment and treatment planning
• Therapeutic interventions to improve mobility and function
• Patient and caregiver education techniques
• Clinical documentation and compliance with state/facility standards
• Interdisciplinary collaboration and effective communication
• Strong organizational skills to manage caseload and treatment goals
