Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (day); some weekend coverage required; emergency on-call as needed

Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Conduct comprehensive OASIS assessments and skilled nursing evaluations in patient homes
- Perform patient assessments, develop individualized care plans, and monitor clinical progress
- Document thoroughly and accurately in the HealthWyse (Careficient) EMR for each visit
- Coordinate care with hospice, outpatient teams, and community resources to optimize outcomes
- Provide patient and caregiver education to promote independence and safe home care
- Travel between patient homes safely and manage time effectively to meet visit schedules
- Respond to emergency on-call situations as required to support patient safety
- Maintain compliance with facility background checks and agency policies
Qualifications
- Active Registered Nurse (RN) license in good standing (current and unrestricted)
- OASIS assessment experience is required
- Home health and hospice clinical experience required
- BLS certification required and current
- Valid driver's license in good standing and reliable transportation for patient visits
- Comfort working independently in the field and adapting to schedule changes
- Willingness to complete client background check and onboarding requirements
Education
- Associate Degree in Nursing (ADN) required; BSN preferred
- Active RN license
Knowledge & Skills
- Strong knowledge of OASIS and home health documentation standards
- Proficient EMR documentation skills, specifically HealthWyse (Careficient)
- Excellent patient assessment and clinical decision-making abilities
- Experience with outpatient and home-based care workflows
- Strong communication, time management, and care coordination skills
- Ability to provide compassionate patient and family education in the home setting
