Roles & Responsibilities

• Conduct comprehensive OASIS assessments and skilled nursing evaluations in patient homes
• Perform patient assessments, develop individualized care plans, and monitor clinical progress
• Document thoroughly and accurately in the HealthWyse (Careficient) EMR for each visit
• Coordinate care with hospice, outpatient teams, and community resources to optimize outcomes
• Provide patient and caregiver education to promote independence and safe home care
• Travel between patient homes safely and manage time effectively to meet visit schedules
• Respond to emergency on-call situations as required to support patient safety
• Maintain compliance with facility background checks and agency policies

Qualifications

• Active Registered Nurse (RN) license in good standing (current and unrestricted)
• OASIS assessment experience is required
• Home health and hospice clinical experience required
• BLS certification required and current
• Valid driver's license in good standing and reliable transportation for patient visits
• Comfort working independently in the field and adapting to schedule changes
• Willingness to complete client background check and onboarding requirements

Education

• Associate Degree in Nursing (ADN) required; BSN preferred
• Active RN license

Knowledge & Skills

• Strong knowledge of OASIS and home health documentation standards
• Proficient EMR documentation skills, specifically HealthWyse (Careficient)
• Excellent patient assessment and clinical decision-making abilities
• Experience with outpatient and home-based care workflows
• Strong communication, time management, and care coordination skills
• Ability to provide compassionate patient and family education in the home setting
