Roles & Responsibilities

• Provide skilled evaluation and evidence-based treatment plans for residents
• Deliver hands-on therapy, therapeutic exercises, and functional training
• Document progress clearly and timely to support care plans and transitions
• Collaborate with nursing and interdisciplinary team members to optimize outcomes
• Educate residents and families to maximize independence and safety
• Participate in readmissions reduction and discharge planning efforts

Qualifications

• Current license to practice Physical Therapy in Texas (required)
• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (see education)
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Experience or comfort working in skilled nursing / long-term care settings preferred
• Strong documentation skills and ability to work collaboratively with care teams
• Ability to complete background checks and facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy accepted

Knowledge & Skills

• Skilled in assessment of mobility, balance, strength, and functional performance
• Proficient with restorative and skilled nursing therapy approaches
• Knowledge of ADL training, fall prevention, and assistive device recommendations
• Clear, professional communication with residents, families, and staff
• Competent clinical documentation and electronic health record use
• Problem-solving mindset and patient-centered care orientation
