Roles & Responsibilities
- Provide skilled evaluation and evidence-based treatment plans for residents
- Deliver hands-on therapy, therapeutic exercises, and functional training
- Document progress clearly and timely to support care plans and transitions
- Collaborate with nursing and interdisciplinary team members to optimize outcomes
- Educate residents and families to maximize independence and safety
- Participate in readmissions reduction and discharge planning efforts
Qualifications
- Current license to practice Physical Therapy in Texas (required)
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy (see education)
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Experience or comfort working in skilled nursing / long-term care settings preferred
- Strong documentation skills and ability to work collaboratively with care teams
- Ability to complete background checks and facility onboarding requirements
Education
- Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy accepted
Knowledge & Skills
- Skilled in assessment of mobility, balance, strength, and functional performance
- Proficient with restorative and skilled nursing therapy approaches
- Knowledge of ADL training, fall prevention, and assistive device recommendations
- Clear, professional communication with residents, families, and staff
- Competent clinical documentation and electronic health record use
- Problem-solving mindset and patient-centered care orientation
