Roles & Responsibilities
- Provide competent, compassionate nursing care to residents — administer medications and treatments safely and on schedule.
- Monitor and document vital signs, patient status changes, and responses to treatment with accuracy and timeliness.
- Collaborate with RNs, physicians, and interdisciplinary teams to create and follow individualized care plans.
- Perform wound care, catheter care, and other delegated procedures per facility protocols.
- Support residents with activities of daily living, promoting dignity, independence, and comfort.
- Educate residents and families about care plans and discharge instructions in a respectful, clear manner.
- Maintain precise clinical documentation in the electronic medical record and communicate handoffs effectively.
Qualifications
- Current Licensed Practical Nurse (LPN) license — New Mexico licensure required or ability to obtain prior to start.
- Basic Life Support (BLS/CPR) certification required; additional certifications preferred where applicable.
- Comfortable working in long-term/post-acute care settings; previous LPN experience in similar settings is a plus.
- Strong clinical judgment, attention to detail, and a collaborative attitude.
- Ability to pass facility-required background and health screenings.
Education
- Completion of an accredited LPN/Vocational Nursing program (diploma or certificate).
Knowledge & Skills
- Solid knowledge of medication administration principles and infection control practices.
- Proficiency in conducting focused patient assessments and recognizing changes in condition.
- Familiarity with electronic health record charting and accurate documentation standards.
- Excellent communication and interpersonal skills for working with residents, families, and the care team.
- Time management and prioritization skills for busy shift workflows.
