Roles & Responsibilities

• Provide competent, compassionate nursing care to residents — administer medications and treatments safely and on schedule.
• Monitor and document vital signs, patient status changes, and responses to treatment with accuracy and timeliness.
• Collaborate with RNs, physicians, and interdisciplinary teams to create and follow individualized care plans.
• Perform wound care, catheter care, and other delegated procedures per facility protocols.
• Support residents with activities of daily living, promoting dignity, independence, and comfort.
• Educate residents and families about care plans and discharge instructions in a respectful, clear manner.
• Maintain precise clinical documentation in the electronic medical record and communicate handoffs effectively.

Qualifications

• Current Licensed Practical Nurse (LPN) license — New Mexico licensure required or ability to obtain prior to start.
• Basic Life Support (BLS/CPR) certification required; additional certifications preferred where applicable.
• Comfortable working in long-term/post-acute care settings; previous LPN experience in similar settings is a plus.
• Strong clinical judgment, attention to detail, and a collaborative attitude.
• Ability to pass facility-required background and health screenings.

Education

• Completion of an accredited LPN/Vocational Nursing program (diploma or certificate).

Knowledge & Skills

• Solid knowledge of medication administration principles and infection control practices.
• Proficiency in conducting focused patient assessments and recognizing changes in condition.
• Familiarity with electronic health record charting and accurate documentation standards.
• Excellent communication and interpersonal skills for working with residents, families, and the care team.
• Time management and prioritization skills for busy shift workflows.
