Roles & Responsibilities

• Provide compassionate, resident-centered nursing care as a Licensed Practical Nurse
• Administer medications and treatments accurately and document all care thoroughly
• Perform routine assessments and communicate changes in condition to the care team
• Support residents with activities of daily living to promote dignity and independence
• Collaborate with nurses, therapists, and facility staff to deliver coordinated care
• Maintain clear, timely charting to ensure continuity and quality of care
• Contribute to a positive, safe environment through adherence to policies and best practices

Qualifications

• Current Licensed Practical Nurse (LPN) credential — required
• Valid Vermont LPN license or ability to practice in Vermont (local-only assignment)
• Demonstrated ability to work extended day shifts (7:00 AM - 7:30 PM)
• Strong communication skills and a resident-first attitude
• BLS/CPR certification preferred (or willingness to obtain promptly)
• Previous experience in long-term care or skilled nursing is a plus

Education

• Completion of an accredited LPN (Practical Nurse) program
• High school diploma or GED

Knowledge & Skills

• Medication administration and safe documentation practices
• Basic patient assessment and recognition of clinical changes
• Skilled at supporting activities of daily living with respect and empathy
• Clear verbal and written communication with residents, families, and team members
• Familiarity with electronic charting/EMR systems is helpful
