Roles & Responsibilities

• Provide compassionate, patient-centered nursing care for residents in a post-acute setting
• Perform thorough assessments and communicate changes in patient condition to the care team
• Administer medications and treatments accurately and document according to facility policy
• Collaborate with interdisciplinary team members to develop and implement individualized care plans
• Support admissions, discharges and transfers smoothly to ensure continuity of care
• Educate patients and families on care plans, medications and discharge instructions
• Maintain accurate and timely electronic medical record documentation

Qualifications

• Active Registered Nurse (RN) license valid in Pennsylvania (required)
• Current BLS (Basic Life Support) certification required; ACLS or other advanced certs preferred
• Previous experience in post-acute, long-term care, or skilled nursing settings preferred
• Ability to pass background check, drug screen and meet facility health requirements (immunizations, TB screening)
• Strong interpersonal and communication skills; dependable and professional
• Comfortable working evening/second shift hours and adapting to facility protocols

Knowledge & Skills

• Solid knowledge of nursing assessments, medication administration and wound care
• Familiarity with electronic medical records (EMR) and accurate clinical documentation
• Ability to prioritize tasks in a fast-paced post-acute care environment
• Skilled at patient/family education and building rapport with residents
• Good clinical judgment and ability to escalate concerns appropriately
