Roles & Responsibilities
- Provide compassionate, patient-centered nursing care for residents in a post-acute setting
- Perform thorough assessments and communicate changes in patient condition to the care team
- Administer medications and treatments accurately and document according to facility policy
- Collaborate with interdisciplinary team members to develop and implement individualized care plans
- Support admissions, discharges and transfers smoothly to ensure continuity of care
- Educate patients and families on care plans, medications and discharge instructions
- Maintain accurate and timely electronic medical record documentation
Qualifications
- Active Registered Nurse (RN) license valid in Pennsylvania (required)
- Current BLS (Basic Life Support) certification required; ACLS or other advanced certs preferred
- Previous experience in post-acute, long-term care, or skilled nursing settings preferred
- Ability to pass background check, drug screen and meet facility health requirements (immunizations, TB screening)
- Strong interpersonal and communication skills; dependable and professional
- Comfortable working evening/second shift hours and adapting to facility protocols
Knowledge & Skills
- Solid knowledge of nursing assessments, medication administration and wound care
- Familiarity with electronic medical records (EMR) and accurate clinical documentation
- Ability to prioritize tasks in a fast-paced post-acute care environment
- Skilled at patient/family education and building rapport with residents
- Good clinical judgment and ability to escalate concerns appropriately
