Roles & Responsibilities

• Provide compassionate, patient-centered care within scope of LPN practice and facility guidelines
• Administer medications and treatments accurately while monitoring patient response
• Collaborate with RNs, providers, and the interdisciplinary team to implement and update care plans
• Perform thorough documentation in the electronic health record to maintain continuity of care
• Monitor and report changes in patient condition promptly to ensure patient safety
• Support patients and families with education, emotional support, and clear communication
• Assist with admissions, discharges, and safe patient transfers to keep the unit running smoothly

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing (WV licensure or eligibility required)
• Active BLS/CPR certification required; additional certifications encouraged
• Ability to pass background check, drug screening, and meet facility onboarding requirements
• Previous LPN experience in long-term care, skilled nursing, or similar clinical setting preferred
• Strong communication skills, reliability, and a patient-first attitude

Education

• Graduate of an accredited practical nursing program or equivalent diploma/certificate
• Completion of required state LPN licensure prerequisites

Knowledge & Skills

• Skilled in medication administration, wound care basics, and vital sign assessment
• Comfortable using electronic health records and documenting timely, accurate notes
• Knowledge of infection control practices and safe patient handling
• Ability to prioritize tasks in a fast-paced clinical environment
• Team-oriented mindset with good problem-solving and patient advocacy skills
