Roles & Responsibilities
- Provide compassionate bedside nursing care to residents and patients with professionalism and heart
- Administer medications and treatments safely in accordance with care plans and facility protocols
- Perform accurate assessments and document changes in condition clearly and promptly
- Collaborate with nurses, providers, and support staff to deliver coordinated, person-centered care
- Educate patients and families about care plans, medications, and self-care strategies
- Maintain a clean, safe environment and follow infection control and safety procedures
- Adapt quickly to per diem assignments while maintaining continuity and quality of care
Qualifications
- Active Licensed Practical Nurse (LPN) license in New York required
- Current Basic Life Support (BLS/CPR) certification required or willing to obtain
- Previous LPN experience in long-term care, skilled nursing, or similar settings preferred
- Comfortable with electronic medical records and accurate clinical documentation
- Ability to pass background check, drug screen, and required health screenings
- Professional, reliable, and able to thrive in a collaborative team environment
Education
- Completion of an accredited practical nursing program (LPN/LVN diploma or certificate)
Knowledge & Skills
- Strong medication administration and charting skills
- Clinical assessment of vital signs and basic patient condition changes
- Familiarity with wound care, ADL support, and common long-term care needs
- Clear, compassionate communication with patients, families, and team members
- Time management and prioritization in fast-paced per diem assignments
- Comfort using EMR systems and following facility protocols
