Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
- Administer medications and treatments safely and accurately according to plan of care
- Monitor and document vital signs, patient status, and response to interventions
- Collaborate with RNs, therapists, and the interdisciplinary team to deliver coordinated care
- Assist with ADLs, mobility, and restorative nursing activities to promote independence
- Report changes in condition promptly and participate in care planning meetings
- Maintain accurate, timely charting in the facility’s documentation system
Qualifications
- Valid Licensed Practical Nurse (LPN/LVN) license in Massachusetts (required)
- Current BLS/CPR certification (or willingness to obtain)
- Experience in long-term care, skilled nursing, or rehab preferred
- Strong communication skills and ability to work well within a care team
- Current immunizations and ability to meet facility health requirements
Education
- Graduate of an accredited LPN/LVN program
- High school diploma or equivalent
Knowledge & Skills
- Comfortable with medication administration and wound care basics
- Skilled at resident assessment, documentation, and care planning
- Familiarity or willingness to learn electronic medical record (EMR) systems
- Good clinical judgment, time management, and multitasking in a busy environment
- Compassionate approach to resident-centered care and restorative nursing
