Roles & Responsibilities
- Provide compassionate, direct patient care including assessments and treatments
- Administer medications and treatments safely and accurately
- Monitor and document patient vital signs, responses, and progress
- Collaborate with the interdisciplinary team to coordinate excellent care
- Maintain accurate, timely clinical documentation in the patient record
- Educate patients and families about care plans, discharge instructions, and safety
- Advocate for patients and contribute to a positive, supportive care environment
Qualifications
- Active LPN or RN license appropriate for the role (Class: LPN | RN)
- Current Basic Life Support (BLS) / CPR certification (or willing to obtain)
- Comfortable working in a long-term care/clinical facility environment
- Strong communication, teamwork, and patient-centered mindset
- Ability to pass required background checks and facility onboarding
Education
- LPN: Diploma or vocational nursing certificate (as required for LPN licensure)
- RN: Associate Degree in Nursing (ADN) or BSN preferred (per RN licensure requirements)
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment and vital signs monitoring
- Basic wound care and infection control principles
- Familiarity with electronic health records and accurate charting
- Time management, prioritization, and teamwork in a clinical setting
- Patient and family education with empathy and clarity
