Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents, focusing on safety and dignity
• Administer medications and treatments according to physician orders and facility protocols
• Monitor patient status, document changes, and communicate effectively with the care team
• Perform assessments and report concerns promptly to RN leadership
• Support admissions, discharges, and transitions with clear, patient-centered coordination
• Assist with wound care, IV support (if in scope), and other delegated clinical duties
• Contribute to a positive, team-oriented environment and mentor less experienced staff when appropriate

Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing (required)
• Basic Life Support (BLS/CPR) certification — or willingness to obtain
• Experience in long-term care, med-surg, or similar clinical setting is a strong plus
• Ability to pass background and health screenings per facility requirements
• Strong communication, clinical judgment, and teamwork skills

Education

• Completed LPN diploma or equivalent vocational nursing program

Knowledge & Skills

• Solid medication administration and documentation practices
• Familiarity with patient assessment and care planning
• Comfortable with electronic charting and standard clinical equipment
• Good time management during busy overnight shifts
• Collaborative approach to patient-centered care and interdisciplinary communication
