Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and basic nursing treatments
• Perform thorough patient assessments and report changes to the interdisciplinary team
• Collaborate with RNs, providers, and support staff to create and follow individualized care plans
• Document accurately and timely in the patient record to support continuity of care
• Support patient mobility, comfort, and activities of daily living with dignity and respect
• Educate patients and families on care plans, medications, and follow-up needs
• Follow facility policies, safety protocols, and infection control standards at all times

Qualifications

• Current and active LPN license (Licensed Practical Nurse) — Maine licensure preferred or eligible
• Basic Life Support (BLS) / CPR certification required (or willing to obtain)
• Comfortable working within a team-focused clinical environment
• Good communication skills and a patient-centered attitude
• Ability to pass background checks and any required health screenings

Education

• Graduate of an accredited Practical Nursing program (LPN certificate or equivalent)
• High school diploma or GED required; additional nursing education valued

Knowledge & Skills

• Medication administration and basic nursing procedures
• Patient assessment and monitoring
• Fundamental wound care and infection control practices
• Clear documentation and charting skills (electronic medical records experience a plus)
• Interpersonal skills for working with patients, families, and care teams
• Time management and the ability to prioritize in a busy clinical setting
