Roles & Responsibilities

• Evaluate patients in their homes and develop individualized, goal-oriented therapy plans
• Deliver evidence-based interventions to restore mobility, strength, and function
• Educate patients and families on exercises, safety strategies, and long-term wellness
• Document thoroughly and timely in the electronic health record and mobile charting system
• Collaborate with interdisciplinary colleagues and physicians to coordinate care
• Adjust care plans based on patient progress and clinician/physician feedback
• Support discharge planning and guide patients toward independence

Qualifications

• Licensed Physical Therapist (PT) in the state of New Mexico
• Minimum 1 year of clinical experience (home health experience preferred)
• Strong patient communication, documentation, and problem-solving skills
• Self-motivated and comfortable working independently in patients' homes
• Proficient with electronic health records and mobile charting platforms
