Roles & Responsibilities
- Provide skilled physical therapy evaluations and develop individualized treatment plans focused on function and mobility
- Deliver one-on-one therapy and group sessions that improve patient independence and quality of life
- Document progress clearly and timely in the facility’s EHR to support continuity of care
- Collaborate with nursing, OT, SLP and interdisciplinary teams to coordinate holistic care
- Educate patients and families on home exercise programs and safe mobility strategies
- Monitor outcomes and adjust treatment plans based on patient response and goals
- Support the facility’s quality and safety initiatives, including fall prevention and safe transfers
Qualifications
- Current PT license in New Mexico or eligible for expedited licensure
- Doctor of Physical Therapy (DPT) preferred; new graduates are welcome
- Current BLS/CPR certification or willingness to obtain
- Comfortable working in a skilled nursing facility setting; SNF experience is a plus but not required
- Strong documentation skills and basic familiarity with electronic health records
- Ability to pass required background checks and immunization screenings
- Team-oriented attitude with excellent communication and patient teaching skills
Education
- Doctor of Physical Therapy (DPT) preferred
- Bachelor’s in Physical Therapy or related degree accepted where applicable
- New graduates with DPT considered
Knowledge & Skills
- Skilled in gait training, therapeutic exercise and functional mobility interventions
- Knowledge of geriatric rehab, transfers, adaptive equipment and fall prevention
- Proficient in assessment, goal-setting and outcome measurement
- Familiar with SNF regulatory environment and documentation standards
- Strong patient education and caregiver coaching abilities
- Good clinical reasoning, time management and teamwork skills
