Roles & Responsibilities
- Provide therapeutic interventions and assist patients with mobility, balance and functional activities
- Implement care plans under the supervision of a licensed Physical Therapist to help patients meet goals
- Document patient progress accurately and timely in the facility record
- Collaborate with the interdisciplinary team in the skilled nursing facility to deliver coordinated care
- Educate patients and families on safety, home programs and strategies to promote independence
- Assist with transfers, gait training and use of adaptive equipment to maximize function
- Maintain equipment and ensure a safe treatment environment for patients and staff
Qualifications
- Licensed Physical Therapist Assistant (PTA) or eligible for licensure in New Mexico
- BLS/CPR certification or willingness to obtain prior to start
- Comfortable working in a Skilled Nursing Facility setting; experience with geriatric or post-acute populations is a plus
- Clinical skills in mobility training, transfers and therapeutic exercise
- Strong communication skills and ability to work collaboratively on a care team
- Open to clinicians of all experience levels including new graduates
Education
- Associate degree from an accredited Physical Therapist Assistant program preferred
Knowledge & Skills
- Familiarity with rehabilitation techniques and therapeutic modalities used in SNF settings
- Skilled in safe transfer techniques and proper use of assistive devices
- Solid documentation and time management skills
- Ability to assess patient progress and communicate changes to the supervising PT
- Understanding of infection control, safety protocols and patient confidentiality
