Roles & Responsibilities

• Provide therapeutic interventions and assist patients with mobility, balance and functional activities
• Implement care plans under the supervision of a licensed Physical Therapist to help patients meet goals
• Document patient progress accurately and timely in the facility record
• Collaborate with the interdisciplinary team in the skilled nursing facility to deliver coordinated care
• Educate patients and families on safety, home programs and strategies to promote independence
• Assist with transfers, gait training and use of adaptive equipment to maximize function
• Maintain equipment and ensure a safe treatment environment for patients and staff

Qualifications

• Licensed Physical Therapist Assistant (PTA) or eligible for licensure in New Mexico
• BLS/CPR certification or willingness to obtain prior to start
• Comfortable working in a Skilled Nursing Facility setting; experience with geriatric or post-acute populations is a plus
• Clinical skills in mobility training, transfers and therapeutic exercise
• Strong communication skills and ability to work collaboratively on a care team
• Open to clinicians of all experience levels including new graduates

Education

• Associate degree from an accredited Physical Therapist Assistant program preferred

Knowledge & Skills

• Familiarity with rehabilitation techniques and therapeutic modalities used in SNF settings
• Skilled in safe transfer techniques and proper use of assistive devices
• Solid documentation and time management skills
• Ability to assess patient progress and communicate changes to the supervising PT
• Understanding of infection control, safety protocols and patient confidentiality
