Roles & Responsibilities
- Assess and manage complex wounds and ostomies using evidence-based practice, including wound vac therapy and conservative debridement
- Perform thorough ulcer and skin integrity assessments (Braden scale, neuropathic and peripheral arterial evaluations, tunneling/undermining assessments)
- Provide ostomy care and management for colostomy, ileostomy, loop, double-barrel and mucous fistula patients, including equipment selection
- Deliver compassionate patient- and family-centered teaching about wound care, ostomy self-care, and infection prevention
- Document care accurately and efficiently in EPIC, and communicate changes using SBAR
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Perform and interpret relevant procedures and measurements (e.g., ABI) and obtain wound cultures as indicated
- Maintain strict infection control and isolation precautions while promoting patient safety and comfort
Qualifications
- Minimum 5 years of clinical experience in wound and ostomy care (explicit requirement)
- Active RN licensure appropriate for practice in North Carolina (state licensure required)
- Required certifications: BLS and WCC or WOCN
- Traveler experience welcomed — first-time travelers accepted
- Experience in hospital and pediatric settings is required for this role
- Proficiency with Epic charting and SBAR communication
- Flu vaccine required (medical/religious exemptions only); COVID-19 vaccine not required per facility guideline
- Trauma experience preferred (Level I or II preferred); prior charge experience is a plus
Knowledge & Skills
- Advanced wound care: wound vac therapy, vacuum assisted closure, wound cultures, and complex dressing selection (alginates, foams, hydrocolloids, silver products)
- Debridement techniques: mechanical, sharp, autolytic and enzymatic approaches
- Ostomy management: colostomy/ileostomy care, pouch types (Kock, J-pouch), stoma variations and equipment selection
- Risk assessment tools: Braden scale and prevention risk assessment for adults and pediatrics
- Ulcer assessment & management: neuropathic, peripheral arterial, venous ulcers, staging (I–IV), ABI
- Symptom and comorbidity management: infection control, edema, nutrition/hypoalbuminemia awareness, pain and psychosocial support
- Tube feeding and site care: G-tube/J-tube knowledge
- Clinical communication and documentation: Epic, SBAR, clear interdisciplinary collaboration
