Roles & Responsibilities

• Assess, plan, and implement individualized nursing care for residents under RN supervision
• Administer medications accurately and document medication passes
• Supervise CNAs and support staff to ensure continuity and quality of care
• Maintain clear, professional communication with residents, families, and the care team
• Complete thorough documentation in the Electronic Medical Record to support resident care
• Participate in cross-training across nursing departments as needs arise
• Support a safe and respectful resident environment through vigilant clinical practice

Qualifications

• Current California LVN license required
• Skilled Nursing (SNF) experience required — Sub-Acute does not count
• Long-term care experience preferred and highly valued
• Current BLS certification required
• Must pass the nursing medical test prior to placement
• Ability to communicate effectively in English, both verbal and written
• Basic computer skills and comfort using Electronic Medical Records
• Experience supervising CNAs and working collaboratively with multidisciplinary teams

Knowledge & Skills

• Medication administration and safe medication practices
• Resident assessment and individualized care planning
• Skilled Nursing Facility workflows and long-term care best practices
• Documentation and EHR/EMR proficiency
• Basic computer literacy and documentation accuracy
• Strong interpersonal skills for interactions with residents, families, and agencies
• Willingness to cross-train across nursing departments when needed
