Roles & Responsibilities

• Perform in-home assessments to identify functional impairments and therapy needs
• Develop and implement individualized plans of care that promote safety, independence, and mobility
• Deliver hands-on interventions focused on activities of daily living (ADLs) and home safety
• Educate patients and caregivers on adaptive equipment, strategies, and techniques for daily function
• Coordinate care with nurses, physical therapists, aides, and physicians to ensure seamless service
• Document clinical progress accurately in the EMR and adjust plans in response to changes
• Support safe transitions from hospital to home and foster long-term functional wellness

Qualifications

• Licensed Occupational Therapist (OTR/L) with active New Mexico licensure
• Minimum of 1 year of clinical experience; home health experience preferred
• Comfortable working independently in patients' homes and managing your caseload
• Experience with EMR systems and mobile charting tools
• Strong problem-solving, communication, and time management skills
• Ability to provide patient and caregiver education and to work collaboratively with a care team

Knowledge & Skills

• Expertise in ADLs and functional mobility assessments
• Home safety evaluation and fall risk mitigation strategies
• Knowledge of adaptive equipment selection and training
• Skilled clinical documentation and mobile charting
• Care coordination across multidisciplinary teams
• Strong verbal and written communication and patient education skills
• Time management and independent clinical decision-making
