Roles & Responsibilities
- Perform in-home assessments to identify functional impairments and therapy needs
- Develop and implement individualized plans of care that promote safety, independence, and mobility
- Deliver hands-on interventions focused on activities of daily living (ADLs) and home safety
- Educate patients and caregivers on adaptive equipment, strategies, and techniques for daily function
- Coordinate care with nurses, physical therapists, aides, and physicians to ensure seamless service
- Document clinical progress accurately in the EMR and adjust plans in response to changes
- Support safe transitions from hospital to home and foster long-term functional wellness
Qualifications
- Licensed Occupational Therapist (OTR/L) with active New Mexico licensure
- Minimum of 1 year of clinical experience; home health experience preferred
- Comfortable working independently in patients' homes and managing your caseload
- Experience with EMR systems and mobile charting tools
- Strong problem-solving, communication, and time management skills
- Ability to provide patient and caregiver education and to work collaboratively with a care team
Knowledge & Skills
- Expertise in ADLs and functional mobility assessments
- Home safety evaluation and fall risk mitigation strategies
- Knowledge of adaptive equipment selection and training
- Skilled clinical documentation and mobile charting
- Care coordination across multidisciplinary teams
- Strong verbal and written communication and patient education skills
- Time management and independent clinical decision-making
