Roles & Responsibilities

• Provide skilled physical therapy care in patients' homes with compassion and clinical excellence
• Assess mobility, function, and safety risks, then create individualized treatment plans that improve independence
• Deliver hands-on therapy and therapeutic exercise, adapting approaches to each patient's home environment
• Document timely and accurate clinical notes in the electronic record (Epic preferred) and ensure regulatory compliance
• Collaborate with nurses, social workers, and caregivers to coordinate care and support patient-centered goals
• Complete required Trinity specialty competency testing and skills checklists to maintain high-quality care
• Maintain professional licensure, required vaccinations, and pass mandated health screenings

Qualifications

• Active Physical Therapist license (Iowa) or eligibility for licensure; license/certificate verification required
• BLS (Basic Life Support) certification required
• Home health clinical experience preferred but motivated new-to-home-health clinicians are welcome
• Ability to complete Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Willingness to complete required personal health screenings and immunizations listed by the agency
• Strong communication skills and ability to collaborate with interdisciplinary teams
• Comfort with electronic documentation; Epic experience preferred but not required

Knowledge & Skills

• Skilled in physical therapy assessments, mobility training, and ADL-focused interventions
• Knowledge of home health regulations, safety assessments, and discharge planning
• Proficiency or willingness to learn Epic for documentation (preferred)
• Strong clinical reasoning, patient education, and caregiver training skills
• Familiarity with infection control, documentation standards, and competency testing processes
