Roles & Responsibilities

• Conduct thorough in-home physical therapy evaluations and deliver patient-centered treatment plans
• Perform regular patient visits (typically 5-6 patients per day) and adjust care based on progress
• Document all clinical care and progress notes accurately using the HCHB system
• Coordinate care with nursing, hospice, and interdisciplinary team members to ensure continuity
• Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs
• Manage scheduling and travel between patient homes efficiently while maintaining clinical quality
• Maintain professional standards, infection control, and adherence to unit guidelines and policies

Qualifications

• Licensed Physical Therapist (current, or eligible to obtain state licensure)
• Minimum 2 years of experience, with home health experience required (therapy positions)
• Comfortable working with hospice and home health patient populations
• Experience with documentation systems, specifically HCHB, is strongly preferred
• Reliable vehicle for patient visits and ability to travel within Franklin/Macon County
• Strong communication, assessment, and care-planning skills

Knowledge & Skills

• Hands-on home health physical therapy assessment and treatment skills
• Proficient documentation in HCHB or similar electronic home health platforms
• Time management and scheduling skills for efficient patient routing
• Ability to collaborate with nursing and interdisciplinary teams, including hospice
• Familiarity with home safety assessments and adaptive equipment recommendations
• Comfort working independently in community/home settings
