Roles & Responsibilities
- Evaluate and assess residents to create individualized, goal-driven treatment plans
- Deliver skilled therapeutic interventions and adjust plans based on progress
- Document thoroughly and timely, meeting facility and CRG documentation standards
- Collaborate with nursing, restorative staff and the interdisciplinary team to optimize patient outcomes
- Meet productivity expectations (90%) while maintaining high-quality care
- Maintain professional presence on-site during assigned shifts to secure guaranteed hours
- Adhere to facility dress code (black scrubs) and follow CRG policies and procedures
Qualifications
- Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT); ability to obtain New Mexico license required
- 1-3 years of clinical experience preferred; open to clinicians with at least 1 year of experience
- Experience working in a Skilled Nursing Facility (SNF) or post-acute care setting preferred
- Current BLS/CPR certification (or willing to obtain) and any state-required credentials
- Strong documentation skills and ability to meet productivity expectations
- Excellent communication and teamwork skills, with a patient-centered approach
Education
- Doctor of Physical Therapy (DPT) or equivalent professional PT degree
Knowledge & Skills
- Skilled in resident assessment, functional mobility and therapeutic exercise
- Familiarity with SNF workflows, Medicare coverage basics and discharge planning
- Proficient with clinical documentation and electronic medical records
- Ability to prioritize caseloads, collaborate with interdisciplinary teams and mentor support staff
- Strong clinical reasoning, manual therapy techniques and fall-prevention strategies
