Roles & Responsibilities

• Assess patients’ physical function and diagnose movement dysfunction through observation and clinical evaluation
• Develop and implement individualized treatment plans to restore mobility and reduce pain
• Coach and educate patients on therapeutic exercises, stretches, and safe movement strategies
• Deliver hands-on techniques such as manual therapy and therapeutic exercise to promote healing
• Maintain accurate, timely documentation of care plans, progress notes, and treatment outcomes
• Communicate patient status and progress with families, providers, and the interdisciplinary care team
• Collaborate with speech and occupational therapists and other clinical staff to optimize patient outcomes
• Participate in departmental meetings, in-services, and professional development activities

Qualifications

• Master’s Degree from an accredited physical therapy program
• Current Physical Therapist state license (or ability to obtain prior to start as required)
• Current Basic Life Support (BLS) certification or ability to obtain within 6 months of hire
• Experience providing direct patient care in a long-term care or hospital setting
• Strong documentation skills and excellent verbal communication
• Comfort working primarily outpatient with some inpatient responsibilities (approximately 90% outpatient, 10% inpatient)
• Team player with demonstrated ability to collaborate across disciplines

Education

• Master’s Degree in Physical Therapy from an accredited program
