Candidate Pay Rate: $36 per hour

Roles & Responsibilities
- Provide compassionate, hands-on patient care including medication administration and basic treatments
- Assess and monitor patient condition, reporting changes and escalating concerns to the care team
- Document care accurately and promptly in the patient chart to ensure continuity
- Support wound care, IV maintenance as appropriate, and other skilled nursing tasks within LPN scope
- Collaborate with RNs, physicians, and multidisciplinary teams to deliver coordinated care
- Educate patients and families on care plans, medications, and discharge instructions
- Maintain a safe, clean environment and follow facility policies and infection-control protocols
- Contribute to a positive team culture by mentoring peers and communicating effectively
Qualifications
- Current, active LPN (Licensed Practical Nurse) license in New Hampshire or eligible to obtain state licensure
- Graduate of an accredited practical nursing program (certificate or diploma)
- Current BLS/CPR certification (facility may require additional certifications)
- Clinical experience as an LPN preferred; experience in med-surg, long-term care, or similar settings is a plus
- Ability to work assigned rotating schedule including weekends
- Ability to pass required background checks and health screenings per facility policy
Education
- Diploma or certificate from an accredited Practical Nursing program (LPN)

- Strong medication administration and documentation skills
- Solid patient assessment and vital-signs monitoring
- Effective communication with patients, families, and interdisciplinary teams
- Time management and prioritization during busy, long day shifts
- Infection control, safety practices, and basic wound care
- Adaptability to different unit workflows and electronic health record systems
