Roles & Responsibilities
- Administer medications and treatments safely and accurately following facility policy
- Provide compassionate direct care to residents, promoting dignity and comfort
- Monitor and document patient status, reporting changes to the interdisciplinary team
- Assist with care planning and collaborate with RNs, CNAs, and other staff for coordinated care
- Perform timely and thorough documentation in the medical record
- Support residents and families with clear communication and patient education
Qualifications
- Current LPN license in Vermont (required)
- Previous LPN experience in long-term care, skilled nursing, or rehab settings is a plus
- Must be local only - this position requires you to live locally
- Must be available to work both weekend days as part of the schedule
- Must have reliable personal transportation
- Ability to pass background checks and required facility screenings
Education
- Completion of an accredited LPN/LVN program and active Vermont LPN license
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Familiarity with long-term care nursing practices and resident-centered care
- Clear, professional communication with patients, families, and care teams
- Good documentation and electronic charting skills
- Ability to prioritize tasks in a fast-paced care environment
- Team-oriented mindset and a calm approach to resident care
