Roles & Responsibilities

• Provide safe, compassionate nursing care to residents, using sound clinical judgment and a patient first approach
• Administer medications and treatments accurately while following facility policies and physician orders
• Perform focused assessments and communicate changes in patient condition promptly to the care team
• Document care and observations clearly and timely in the patient record
• Collaborate with nurses, therapists and support staff to develop and implement individualized care plans
• Support residents and families with education and emotional reassurance during transitions of care

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania or compact/eligible for Pennsylvania licensure
• Completion of an accredited practical nursing program
• Current Basic Life Support (BLS) certification or willingness to obtain prior to start
• Clinical experience in long term care or similar settings preferred
• Strong communication skills, reliability, and a team oriented mindset

Education

• Completion of an accredited Practical Nursing program
• High school diploma or equivalent

Knowledge & Skills

• Skilled in medication administration and basic wound care
• Good clinical assessment and charting skills using electronic medical records
• Ability to prioritize tasks in a fast paced environment
• Comfortable working as part of an interdisciplinary team and following standing orders
• Strong interpersonal skills for patient and family interactions
