Roles & Responsibilities
- Provide compassionate bedside nursing care to residents at Laconia Rehab Ctr, focusing on safety and dignity
- Administer medications and treatments accurately and document according to facility protocols
- Monitor and assess patient conditions, reporting changes promptly to the care team
- Collaborate with RNs, therapists, and support staff to implement individualized care plans
- Assist residents with activities of daily living and promote independence whenever possible
- Maintain clear, timely charting in the facility’s documentation system to ensure continuity of care
- Support infection control practices and maintain a safe, clean environment for residents
Qualifications
- Active Licensed Practical Nurse (LPN) license — state licensure appropriate for assignment
- Current Basic Life Support (BLS) certification or willingness to obtain upon hire
- Comfortable working overnight shifts and providing hands-on resident care
- Strong communication skills and ability to work collaboratively in a multidisciplinary team
- Previous experience in long-term care, rehabilitation, or skilled nursing settings is a plus
Education
- LPN diploma, certificate, or equivalent vocational nursing program completion
Knowledge & Skills
- Medication administration and documentation best practices
- Resident assessment, vital signs, and clinical observation skills
- Familiarity with long-term care and rehab nursing routines
- Proficient in clear, concise charting and electronic medical records
- Strong interpersonal skills for working with residents, families, and care teams
- Infection prevention, wound care basics, and safe patient handling awareness
